Debit card for a 13 year old?

Where can I get a debit card that has no monthly, annual, fees or charges you to buy things or add money? I have plenty of money ($500) and one of my parents is willing to be a co-signer. Debit card, not credit. I want no fees for everyday type of stuff, using my money adding my money whatever.
